Adoptive Parent Forms

Application for Home Services only: If you are applying for our agency to complete a home study only, you do not need to submit the Pre-Application but may go directly to submitting the Adoptive Home Application with the application fee of $250. Upon receipt of your application, one of our staff members will contact you to proceed with scheduling visits to begin the home study.

FLS Domestic Infant Adoption Program:  The first step in applying to our domestic infant adoption program is to complete the Pre-Application Form and Statement of Faith which are used as screening tools to see if you meet the basic criteria of our program. There is no fee to submit these forms and no obligation to continue with the full application process.

Once these documents are received by our office, you will receive an email response from our office to let you know if we recommend that you continue with submitting a Adoptive Home Application, Getting To Know You, and Budget Form for review by the Board of Directors.

Once you submit the Adoptive Home Application, Getting To Know You, and Budget Form, it will be reviewed by the agency’s Director and the Board of Directors for approval. There is a $250, non-refundable, application fee due at the time of application submission. It typically takes 2-4 weeks for a decision to be made and you will receive formal notification of the decision, by letter, from the agency.

After an application has been approved, you will be invited to the next Adoption Training Weekend offered to complete the prerequisite training to enter into our adoption program.

Virginia Home Studies Only: For Virginia residents completing a home study with FLS, the following form and instructions will be used to search the Virginia Child Abuse and Neglect Central Registry. Do not complete these forms until you have received a home study packet from our agency that instructs you to complete them.
